Stop in the lobby for a Blood Pressure Check provided by Central Connecticut Health District staff. What's a Social Determinants of Health (SDOH) screening? We ask a few questions about things that can affect your health as you age, such as getting to appointments, affording medications, having enough food, staying connected with others, and feeling safe at home.
